In completion of this form, I understand that I am responsible for setting up and tearing down for my activity. This includes putting equipment away, picking up and taking out trash, sweeping and vacuuming. I understand that church decorations must be left and not changed without proper approval. I also understand that a fee may be charged to this usage, payable at least a week before event.

Upon submission, you will receive a confirmation e-mail that your request has been received. Expect an approval/disapproval response within 2-3 business days.
